Writing the Incident Report The purposes of an incident report include the following: Ø Cover the liabilities Ø Objectively report what happened Ø List all relevant information Ø Recommend what you think should be done as a result of the incident The headings in an incident report are these: Introduction Discussion Incident Cause Result Conclusion and Recommendations ……………………………………………………………………………………………… THE INCIDENT When you arrived at work one morning, you found the front doors unlocked even though it was only 7 a.m. and the plant doesn’t open until 8 a.m. Usually Barney Fife, the security guard, is at his post immediately inside the front door, but he was not there on that morning. You began to think something was up. As you looked around the lobby, you called Barney’s name, but there was no answer. Everything else seemed to be all right, so you went to the elevators and pressed the button to call the elevator to the first floor. As you were waiting for the elevator, you heard music coming from an office down the hall, so you decided to investigate. You went to Office 102 and found that the door was unlocked. When you opened the door, you found a radio playing, but no one was in the room. Just to be sure, you called for Barney again, but there was no answer. You decided that you would not search the office but would instead call your supervisor when you got to your work station. Just then, as you were still standing in front of Office 102, you heard the ding of the elevator, signaling that it had arrived on the first floor. As you hurried to catch the elevator, you arrived at the open elevator door and saw Barney lying unconscious but still breathing on the floor. You gasped. Just as you turned to go to Barney’s desk to call the police and an ambulance, you saw … TASK Write an incident report in memo format about what happened. To do that, you will have to finish the story above. Pay attention to the guidelines for good memo and report design.
